IMPORTANT CHANGES IN HEALTH REQUIREMENTS FOR THE 2021/2022 SCHOOL YEAR:
All child health examination forms, including records of immunizations, dental, vision records and medication forms are to be presented to the school before or at Packet Pick Up (August 18th, 2021). If a family is not in compliance by this day, the child(ren) will not receive their classroom placement or packet from school until the child presents proof of having had the necessary documents. Please allow yourself ample time to complete these requirements by making appointments now to ensure your child can be seen by their healthcare provider prior to this date. If your student requires an epi-pen or inhaler, the Medication Administration Form must also be completed by this deadline.
Physical Exams/Immunization Record
All children entering kindergarten or 6th grade as well as all children enrolling at Christ the King for the first time (regardless of grade), are required to have a State of Illinois Certificate of Child Health Examination Form which includes immunization record. Parents, please fill out the health history portion (page 2), sign, and date. This includes meningococcal vaccine for all 6th grade students.
Any student, regardless of grade, that plans on playing sports at Christ the King needs a sports physical. This requirement can be fulfilled on The State of Illinois Certificate of Child Health Examination Form by having your child’s doctor circling “yes” for interscholastic sports on the last page of the form. An independent sports physical form is also available.
All children entering Kindergarten, 2nd or 6th grade as well as all children enrolling at Christ the King for the first time (regardless of grade) will need a dental examination with completed Illinois Dental Form.
All children entering Kindergarten as well as all children enrolling at Christ the King for the first time (regardless of grade), will have an eye exam with completed Illinois Eye Exam Report.
Allergy Action Plan
Children who require an epi-pen or antihistamine for allergic reactions must have their Illinois Food Allergy Emergency Action Plan and Treatment Authorization documentation prior to Packet-Pick-Up (August 18th, 2021). If your child currently has an epi-pen or antihistamine at school we will store them through the summer, but will need the new documentation which includes the Action Plan along with the Medication Administration Form. Please contact the office if you would like your child’s epi-pen or medication returned on the last day of school.
Asthma Action Plan
Children who require an inhaler for asthma must have a documented Asthma Action Plan and Medication Administration Form from their pediatrician prior to Packer-Pick-Up (August 18th, 2021). If your child currently has an inhaler at school we will store them through the summer but will need the new documentation. Please contact the office if you would like your child’s inhaler returned on the last day of school.
